Holiday Gift Registration Form 2026

Welcome to Claus for a Cause and Angel Tree Sign ups!

This form is for registering youth aged 0 to 18 (or 18 at the beginning of this academic school year) who reside in Jefferson and surrounding counties. While we aim to provide gifts for each family this holiday season, we ask that you submit wish lists as early as you can, and try to be as realistic as possible. Back to Play, Toys for Tots, and The Salvation Army look forward to contributing to your holiday season.

Please fill out the form by section and for each child needed. At the end, PLEASE CONFIRM your pick up preferences. You will have the option to have your responses sent to your email for verification. 

💡 Why do we ask these questions? Back to Play relies on grants and local donations to keep "Claus for a Cause" 100% free. The anonymous totals from this form allow us to prove community needs to grantors without ever sharing your name or private details.

Please list the first and last names of each member in your household, including children. DOB is helpful but not required if unknown. This is strictly for data/records with Salvation Army and does not impact eligibility for receiving gifts.
