How to set fields as protected health information?

Google Forms does not support PHI fields, but you can use Formesign addon to mark the fields as protected health information.


How to mark fields as protected health information in Google Forms?


  • Open your form in Google Forms

  • Click on the addon icon

  • Click Formprefill

  • Click Create prefill links

  • Click Proceed

  • Share page will open in a new tab

  • Click Edit

  • In the Edit page, click on the gear icon next to the field

  • Field settings page will be displayed

  • Mark the field as Protected Health Information

  • Click Save
